Health and Care Innovations Limited are the representative of the University of Exeter in the delivery of the REACH-HF programme and body operating the Digital REACH-HF platform on behalf of the University of Exeter.

The D-REACH app, developed by the University of Exeter, is the digitised version of the REACH-HF intervention (developed by a collaboration between the University of Exeter, NHS Lothian, Royal Cornwall Hospital Trust and Prof. Patrick Doherty).
The D-REACH app can be used by healthcare professionals and their patients as a tool to support a healthier lifestyle, track levels of exercise and wellbeing and self-management of symptoms for patients suffering from heart failure.
By viewing, using and/or accessing the D-REACH app, you acknowledge and agree that this website/app is:
(i) not certified as a medical device
(ii) not intended for any diagnostic use and
(iii) intended to be used solely for non-commercial, medical training and performance improvement purposes.
At no time shall D-REACH be used to diagnose, treat or cure a human being’s condition or in a life-saving situation; support professional medical decisions, diagnosis or treatments, or replace any diagnosis, recommendation, advice, treatment or decision by an appropriately trained and licensed physician.
Liability
To the fullest extent permitted by law we, and third parties from time to time connected with us, hereby expressly exclude:-
All conditions, warranties and other terms which might otherwise be implied by statute, common law or the law of equity;
Any liability for any direct, indirect or consequential loss or damage (including damage of any kind, however arising and whether caused by tort (including negligence), breach of contract or otherwise (even foreseeable) incurred by you in connection with our services or in connection with the use, inability to use, or results of the use of our site (including the Content and Material) any websites linked to it and any content posted on it.

If defective digital content that we have supplied damages a device or digital content belonging to you and this is caused by our failure to use reasonable care and skill, we will either repair the damage or pay you compensation. However, we will not be liable for damage that you could have avoided by following our advice to apply an update offered to you free of charge or for damage that was caused by you failing to correctly follow installation instructions or to have in place the minimum system requirements advised by us.
We do not exclude or limit in any way our liability to you where it would be unlawful to do so. This includes liability for death or personal injury caused by our negligence or the negligence of our employees, agents or subcontractors and for fraud or fraudulent misrepresentation.
